What would be a fair price for a CIB Soldier Blade in excellent condition? A price that would be fair to both seller and buyer?

Judging by ebay prices, I'd say $65 or so (it's gone up a ton in the last year). Without the box, $45 would be about right, but that hunk of cardboard adds quite a bit to the value.

FWIW, I bought a CIB copy off eBay 6-8 months ago for $40. It was just a regular auction not a BIN.
