ISO: High Quality scan of original Ys Book I & II rear

Hi all,  Ive looked online and cannot find a good enough scan of the all black rear CD inlay/insert for the original Ys book I and II (US)  I have the duo pack in, and would like to add the cd insert, I can print it, I just need a good scan.

The one I found online, does not have the yellow bar at the top.  Yes I know its just black but I need one nonetheless.
