Yeah I paid aprox 40 for my pc engine Cotton.  Really is no difference in the two.  Well none worth nothing anyway.  You can select English text for the game too.

Also you can hear the dialog in the cutscenes. So unless you're a collector, there's no need to pursue the rare US version.

So, exactly what is the difference between the Japanese-released version and the U.S.-released version other than the U.S.-released version being much rarer?

The japanese version has voice acting for the cutscenes.
