Dracula X is awesome, but challenging.  If you're a fan onf the NES Castlevania games, this is the cream of the crop.  If you're a fan of Symphony of the Night style gameplay, this game can be very frustrating.
Spriggan is probably my favorite CD game though it's import only.  Really amazing shooter.
Gates of Thunder actually has 4 games on the disc.  GoT, Bonk 1 and Bonk 2 as well as Bomberman.  GoT is an awesome shooter with a rockin' soundtrack and worth the purchase even if you don't want the other games.
Ys Books 1 & 2 is a great action RPG.  Spectatular music.  This is truly one of the standout games for the system, imo.
Buster Brothers is a great 2-player co-op game.  It doesn't get a lot of fanfare but I think it's a lot of fun.
Riot Zone is along the same vein as Final Fight or Streets of Rage, only it's one player.  It would have been awesome to be able to play through this with a buddy, but it's still decent as a single player game.
Sapphire is an outstanding shmup, but it requires an Arcade Card. 
Sherlock Holmes 1 was another game I really enjoyed playing (and the fmv was impressive at the time though it's laughable now) but doesn't get much fanfare. Part 2 or J.B. Harold Murder Club didn't hold the same interest to me as the first SH game, though I don't think I've given a fair shake to the J.B. Harold game yet.
Lords of Thunder is another rockin' shmup.   Very challenging, but the graphics an music make the punishment very rewarding.
Star Parodier is an awesome cute 'em up which is kind of a spiritual successor in another dimension to Super Star Soldier.  You can play as Bomberman, a Star Soldier spaceship, or a PC Engine.  Lots of humor, great gameplay, fun game!

I keep hearing Dungeon Explorer II is outstanding, though I haven't really dug my teeth into it yet.  These days I prefer a game I can play for 30 minute chunks, so most shooters fit the bill perfectly.  RPGs are a little harder for me to get into these days.

In addition to the other fine suggestions, here's a few of my favorites:

Implode - cheap puzzler with cool, fitting tunes
Camp California - under appreciated platformer with nice variety but somewhat wonky controls
Splash Lake - wonderful puzzley action, even better with a friend
Gulliver Boy - worth looking at just for the HuVideo
Ys III - Wanderers From Ys - I & II and IV are mentioned but not III... what's wrong with you f*ckers?!?
Kabuki Itouryodan - beautiful and original fighter, but requires the Arcade Card
Götzendiener - clunky and unfinished, but everyone should have this title anyway
Rainbow Islands - all the cool people love Bubble Bobble
Pop 'n' Magic - single screen action similar to Parasol Stars / Rainbow Islands
Faceball - you know y'all want it
U.S. Collection: 97% complete    155/159 titles

I see them pop up every now and then on eBay for reasonable prices. Expect to pay between $40-$50.

These are my most-played CD games.  I think it's impossible to determine the "best" games; the whole U.S. library of CD games are pretty solid.


Honarable mentions:
Shadow of the Beast - ear/eye candy, otherwise average.
Prince of Persia - Underrated only because there are so many other ports.
Godzilla - My favorite 2-button tournament fighter.
It Came from the Desert - everyone should have at least one early FMV game.

The import Dynastic Hero is about $20 compared to $300+ for the U.S. version.  Buy it, play through the Genesis or SMS version first, and you'll be able to play it without a walkthrough.
Bonk 3 is a good two-player, but I don't recommend it because it not as good overall as the earlier games in the series and the hucard is cheaper (and the j-hu cheaper still, a consideration for the OP).
Rainbow Islands is good, but it's expensive and a one-player, vertical-scrolling platformer doesn't satisfy my Bouble Bubble craving.  I'd recommend sticking to Parasol Stars and Don Doku Don unless you're a BB fanatic.
I don't know what the attraction is with Spriggan.
There are tons of good PCE RPGs if you're not intimidated by the language barrier.

Yeah, they're two totally different games. Like Arkhan said, the first is a vert and the second (sequel) is a horizontal.
